We take your health very seriously. Many medical conditions can have a profound effect on your dental care and long term health, and vice versa.
It is vital for all patients attending LCIAD to complete our confidential Medical History Questionnaire (MHQ) before their first appointment. This form will take around 15 minutes to complete. You can start the form and return to it later to complete by using the Save button.
If you are a new patient to LCIAD, please ensure that we receive your completed form at least 72 hours before your consultation so that we may prepare properly for your first visit. Please also complete our one-off Dental Questionnaire before your first consultation.
The Medical History Questionnaire (MHQ) needs to be completed once and updated and your copy digitally signed by you at every visit. Your existing data is maintained so you do not need to complete the whole form again*. A link will be sent before your next appointment which you should keep for future updates.
* It may occasionally be necessary to revise the form structure to a new version as medicolegal requirements change. We may need to update the MHQ every 2-3 years requiring new completion. We appreciate your understanding and co-operation is this becomes necessary.
